start horizontally, and for the 'spikes' (where the bits of inches body goes up or down), you can either make those seperately and tie them on, or make it all in one piece, but it takes a lot of weaving to skip beads between the spikes. i can make a vid if my instructuons arent clear XD

I would start at the two black beads at the end of the worm and make it like a normal charm. It is an even stitch so you won't have to worry about maneuvering the string around a lot so you most likely won't have an issue with the top and bottom halves.

For the antennas you could use a separate piece of string and kinda put the beads on like you would a kandi octopus tentacle and then tie it on. Make sure you tie the string really tight so they don't flop around a bunch

If you start at the middle of the caterpillar where theres a straight line of beads for the first step of the peyote, then do the bottom half...

In the youtube tutorial, if you fast foward to the part of the mask where its not a straight line, it will show how to do the irregular shape like in the caterpillar
